XLHealths unique disease management model goes beyond telephone communications with patients. The companys high-touch approach is based upon the belief that one of the best ways to assess a patients condition is to have medical personnel conduct face-to-face evaluations. XLHealth also guides patient support using its sophisticated predictive modeling technology, which draws from in-depth patient profiles integrated with claims, pharmacy, and lab information.

XLHealth understands the critical role physicians play in the healthcare equation and believes that its disease management and support programs are most effective when they establish strong ties with the primary physician and other treating providers. In 2003, the U.S. Department of Health and Human Services and the Centers for Medicare & Medicaid Services (CMS) awarded XLHealth a demonstration project to test whether disease management improves the health of chronically ill Medicare patients. In less than one year, XLHealth developed a highly effective recruitment process that resulted in the enrollment of 10,000 patients for the program.
