Personal Support Nurses
At the start of the program, your Personal Support Nurse will contact you to arrange an appointment at an XLHealth
center or a home visit. Your nurse will get to know you, and will collect information about your health,
your family, and your lifestyle. This information will be used to help your doctor manage your disease.
Your Personal Support Nurse will be available to answer your questions and you may have periodic visits with your
nurse throughout the course of the program.
Telephone Nurses
The Medicare Health Support Program will assign a Telephone Nurse to support you throughout the program.
These nurses work closely with your Personal Support Nurse, and will call to remind you about important appointments,
talk about your health goals, help you understand your health condition, and discuss anything unusual thats come up
from your assessments and in-home health monitoring.
