Personal Support Nurses
At the start of the program, a Personal Support Nurse will contact the patient to arrange a face-to-face
meeting. The nurse will take the time to get to know that patient and learn about his or her health,
family and lifestyle. This information will be shared with the participants physician to help manage
the participants disease. Throughout the course of the program, the Personal Support Nurse will be
available to answer the participants questions and make periodic home visits or center appointments.
Telephone Nurses
The Medicare Health Support Program has a large team of specially trained nurses to give patients
support over the phone. These nurses work closely with the Personal Support Nurse, and will call
to remind the patient about important appointments, talk about health goals, help the patient understand
his or her illness, and discuss any concerns that come up during face-to-face visits.
