Selected Medicare beneficiaries living with specific chronic health conditions have been invited to participate in the Medicare Health Support Program of Tennessee. The goals of the program are to help beneficiaries better manage their health, adhere to their physicians' plans of care, and reduce their health risks. Medicare Health Support is a program from the Centers for Medicare & Medicaid Services (CMS) provided in Tennessee by XLHealth.

Medicare Health Support is not a substitute for office visits or medical care, nor is it a managed care plan. Medicare Health Support is designed to be a support service that reinforces your instructions to your patients, monitors their condition between office visits, and helps bring relevant and up-to-date patient information to the physician when it’s needed.

You may have patients who are among the 20,000 Medicare beneficiaries in Tennessee who’ve been invited to participate in this pilot program.
