Program Overview

Welcome to Medicare Health Support.

You may have been invited by Medicare to participate in a new program to help Medicare beneficiaries with chronic health conditions reduce their health risks and improve their quality of life. Medicare Health Support is a program to help you better manage your health, follow your doctor's plan of care, and coordinate your medical care. Participating in Medicare Health Support does not change your Medicare benefits, and you can continue to go to the same doctors and hospitals. Your participation is voluntary, and this new program is available at no cost to you.

With Medicare Health Support you’ll also have your own Medicare Health Support team consisting of your own personal support nurse, a telephone nurse, a local pharmacist and your regular doctor. They’ll provide you with education and support services that will give you the chance to ask questions, help you understand your condition and incorporate healthy habits into your lifestyle. With your approval, family members or caregivers may also ask questions of your support team on your behalf.
