Program Overview

Support to your Patients

The Medicare Health Support Team will provide your participating patients with a wide range of support, including:

  • Visits with a Personal Support Nurse

    Face-to-face visits are scheduled to get to know each patient and monitor their health and medicines between doctor appointments.

  • Follow-Up Telephone Calls

    A Telephone Nurse calls to remind patients about important appointments, answer questions, and discuss any health concerns they may have.

  • Education and Support Services

    On-site education sessions are available, helpful materials are mailed between visits, and one-on-one sessions take place to help participants follow their doctors' plan of care.

  • In-Home Health Monitoring Tools

    If needed, patients are encouraged to monitor their health at home using helpful tools supplied by XLHealth and individualized plans to observe important health signs.

  • Medication Counseling Sessions

    A one-on-one counseling session with a local pharmacist is available to participants to review the beneficiary's prescription regimen and answer any questions they may have about their medications.
