Support to your Patients
The Medicare Health Support Team will provide your participating patients with a wide range of
support, including:
Visits with a Personal Support Nurse
Face-to-face visits are scheduled to get to know each patient and monitor their health and medicines
between doctor appointments.
Follow-Up Telephone Calls
A Telephone Nurse calls to remind patients about important appointments, answer questions,
and discuss any health concerns they may have.
Education and Support Services
On-site education sessions are available, helpful materials are mailed between visits, and one-on-one sessions take
place to help participants follow their doctors' plan of care.
In-Home Health Monitoring Tools
If needed, patients are encouraged to monitor their health at home using helpful tools supplied by XLHealth and
individualized plans to observe important health signs.
Medication Counseling Sessions
A one-on-one counseling session with a local pharmacist is available to participants to review the beneficiary's
prescription regimen and answer any questions they may have about their medications.
