The Medicare Health Support Program helps to improve your daily health and
supports your physicians care plan by providing you with a wide range of support.
These services will be provided by your Medicare Health Support Team whose job is to get
to know you and your specific needs.
Appointments with a Personal Nurse
Your Medicare Health Support Team will schedule face-to-face visits to get to know you and check
on your health and medicines between doctors appointments.
Follow-Up Telephone Calls
Your Telephone Nurse will call you to remind you about important appointments, answer your questions,
and discuss any concerns you may have about your health.
Education and Support Services
Likewise, your Telephone Nurse will be available to answer any questions when you call. Your Support
Team will also host nurse education sessions, mail helpful materials to you between visits, and work
with you one-on-one to manage your medication and nutrition.
In-Home Health Monitoring Tools
If needed, your Medicare Health Support Team will provide you with tools and support to help you and your doctor monitor
important health symptons between doctors visits.
Medication Counseling Sessions
A one-on-one counseling session with a local pharmacist at CVS pharmacy, Ike's and Super D, is available to help you review your
prescriptions and answer any questions you may have about your medications. Selected CVS pharmacies,
Ikes and SuperD drug store locations are available for this service.
